Eligibility
Who may align with an Atlas IPA Participant Provider?

- Enrolled in Medicare Parts A and B
- Medicare is the primary payer
- Not covered under Medicare Advantage or another Medicare managed care plan
- Resident of the United States
- Resident of a county included in the Atlas IPA service area
- For high needs population: one or more mobility-impairing conditions
- For high needs population: at least one significant chronic or serious illness
