Enrolled in Medicare Parts A and B
Medicare is the primary payer
Not covered under Medicare Advantage or another Medicare managed care plan
Resident of the United States
Resident of a county included in the Atlas IPA service area
For high needs population: one or more mobility-impairing conditions
For high needs population: at least one significant chronic or serious illness
